Except where indicated, the following forms are in PDF. All forms can be filled out online or printed and filled out by hand. However, you cannot save these forms or send them by e-mail. If you have any questions about using these forms, please e-mail tariff helpdesk.
Authorization for Additional Charges form
If there are additional charges on the same Information/Indictment, add them to your billing form without getting special authorization and attach a copy of the Information/Indictment.
If there are additional charges on different Informations/Indictments more than 90 days after the interview/assignment date, instruct your client to reapply for legal aid.
To submit your accounts electronically, log in to e-billing. To submit your accounts by mail, fill out the forms online or fill them out by hand, and mail the print-outs to LSS for payment.

Settlement or judgment confirmation form
Contact the Collections Audit Clerk if you learn that a settlement or judgment will be received by your client.
Third party disbursements billing form
This form is for use by third party vendors who have contracted for payment directly with the Legal Services Society.
Vendor/e-services/direct deposit forms
Vendor Application and Change Form (for lawyers not on e-services only)
To get a private bar vendor number, complete the required fields in the form below, save it, then return the printed form to LSS by fax at 604-682-7967 or by e-mail. Once LSS receives the form, your information will be entered into our referral system and you will be issued a private bar vendor number. LSS will send you a Guide to Legal Aid Tariffs with billing forms by post, if required.
If you have to change any of the information you've submitted to us, such as your contact details or referral locations, please notify LSS in writing by fax at 604-682-7967, by e-mail, or send us an update of the following form to the address below.

Vendor Number Reapplication Form
To apply to reactivate your vendor number, or to have conditions on your eligibility to receive referrals removed or modified, complete the required fields in the form below, save it, then return the printed form to LSS by fax at 604-682-7967, by e-mail, or by regular mail to the address below.
If LSS deactivated your vendor number or imposed conditions on your eligibility to receive referrals, you must demonstrate that the reasons for deactivation or conditions have been addressed. Additional information to support your reapplication may be provided by attaching a letter and/or documents.

Lawyer Direct Deposit Form
To have payments directly deposited into your bank account, you must be registered with e-billing. If you are registered, complete the following form and return it by mail or fax to LSS.

List the e-mail address that you want us to use to send notification that your payment was deposited. If this e-mail address is different from what LSS currently has on file, we will retain two e-mail addresses on your account profile.
Note: If you are registered for electronic fund transfer (EFT), payment stubs are posted online; therefore, they are no longer mailed.
If any information changes on your Lawyer Direct Deposit form, please notify the Finance Department in writing by fax at 604-682-7967.
